Lefevre, Medical Lake, on Wednesdays-Thursdays, 5-7 p.m. What to bring: 2015 tax return (if available), valid picture ID, all income statements such as 2016 W-2s, 1098, 1099 forms for yourself and spouse, any forms 1095 - proof of health insurance, social security cards for each family member and working adult, proof of other income from yourself and spouse, childcare provider information and tax ID, property tax information, if home owner, 1098T and educational expenses for students, bank routing and account numbers for checking or savings account.
